Our Model Release

To Kisville Enterprises Inc. & Kisville Photography:

 I acknowledge receipt of value, either monetary or otherwise in return for this release. I acknowledge that Kisville Enterprises Inc. & Kisville Photography intend to act on the information contained herein.

 I hereby, irrevocably give you, your successors, assignees, licensees and anyone acting with your consent, all rights, title and interest, including copyright and other literary property.

Other literary property includes commercial and publication rights (i) in and to any and all depictions of my likeness (whether created or issued as stills, motion pictures, or on video tape or other media known or unknown at this time, in which I may be included, including any reproductions thereof, made in any form and manner. I also grant Kisville Enterprises Inc. & Kisville Photography irrevocable rights to my name (real or fictitious), my voice, my signature, performance, likeness and any biographical material pertaining to me.

All items contained in the above sections shall be collectively referred to as the “Materials”.

You have the absolute right and permission to use, reuse, publish, republish, exhibit, display, print, reprint, distribute and copy in whole or in part any of the Materials with restriction as to Changes, distortions or transformations.

The Materials may be used for editorial, advertising, art, promotion, trade or any other lawful purposes whatsoever in any and media or forms known (including, but not limited to digital and electronic) or unknown at this time.

I hereby waive my right to inspect, approve or object to the Materials, or the editorial, visual, advertising copy or the printed material that may be used in conjunction therewith and to the use to which depictions, likeness or reproductions, or any of the foregoing may be applied and acknowledge that Kisville Enterprises Inc. and Kisville Photography are under no obligation to utilize the Materials.

I hereby release, discharge and agree to save Kisville Enterprises Inc. & Kisville Photography, its successors, assignees, licensees or anyone acting with Kisville Enterprise Inc. & Kisville Photography consent, harmless from any liability by virtue of any blurring, distortion, alteration, optical illusion or use in composite form, whether intentional or otherwise, that may occur or be produced in taking or reproducing the Materials.

I certify that I am 18 years of age or older, and was so on the date the depictions of me to which this release applies were taken or have had this release signed by my parent or guardian. I further certify that any proof of age that I may have supplied in support of this is valid and a true, real reflection of my age and/or birth date.
